Concussion Training Work Around

The CDC HeadsUp site is experiencing extremely high traffic. Avoid the hours of 4:00pm to 8:00pm if possible, but if you are unable to login to the CDC site, here is a workaround for now:

Follow the steps below to read the training transcript and answer the summary questions. Then you can email the answers to the test and someone at the CDC will actually email you your new completion certificate! Just include your LEGAL name in with the answers. 

  1. Click to Open: CDC Concussion Transcript and Post-Test
  2. Send your answers to the Post-Test to jscoville@banyancom.com at the CDC.  Be sure to include your name in the email.  Jana will email your Concussion certificate to you 24-48 hours after receipt. No need to print/scan the test if you don't want to, you can just send her the answers in the text of your email along with your full legal name.
  3. Once you receive your certificate, you need to upload it to your Stack account.   

Please NOTE: Most of you at this step will have already created your Stack account, so to log back in and upload your certificate, just use the link in the Westwood Youth Soccer: Account Information email sent to you when you registered, or if you use the link below ONLY CLICK on the RED "Member Login" BUTTON on the top right of this page to access your account otherwise you will create a duplicate Stack account.

REMINDER: If you took the CDC concussion training last year and have your certificate we will ACCEPT that for this soccer year! You just have to upload it to your Stack account. Follow the instructions above to access your account.
